    Something fatal that could be treated and put off for several years if you were willing to put enough money into it, but will still get you in the end.

    Most cancers. My ex brother an law has been battling throat cancer for years now, but in the end it will probably get him. Been through the chemo 3x now in 7 years, but it wouldn't be hard just to make it come back and win for the sake of a story. Leukemia is very often eventually fatal too, although some people can live decades. And i saw some story on discovery about a parasite that settles in and can take a decade to show.

    AIDS is rarely fatal now - people remain HIV positive and never develop fill blow AIDS...

    Untreated HIV would kill in a few years though from AIDS

    Syphilis could work. It can go dormant, and people used to think it was gone ... and then it came back in a really nasty form. Of course, it's curable nowadays with antibiotics.

    Some forms of brain tumour are very slow growing - taking anything up to fifteen years to get to the stage where they will kill you. And if you're crafty where you site the tumour, it could be one of those tricksy ones that are inopperable. They can also have some interesting effects on a person's behaviour, which you may be able to use...
